Our Care & Support, Direct to Your Door

Through our pioneering Community Care, Royal Freemasons' Outreach program continues to help people stay where they want to be.

Continually at the forefront of aged care innovation, Royal Freemasons pioneered Community Care in the early 1990s, and since 2001, the organisation has become a major provider of care packages across metropolitan Melbourne. The care and support delivered through our various Outreach programs, gives eligible clients the opportunity to continue to enjoy the comfort and independence of living in their own home.

Coordinated programs are delivered by trained staff from Royal Freemasons, rather than agency staff that many other providers rely on, so it's always someone you know who comes to the door. Our programs are individually tailored to each client's need depending on their circumstances and preferences, and include help and assistance with a wide range of personal and household tasks as well as on-going management of care. These programs currently comprise Community Aged Care Packages (CACP), Extended Aged Care at Home (EACH) and special EACH Dementia Packages. These three packages cover clients who would be eligible for low and high residential care, but who choose to remain at home.

Transition Care

To help clients be discharged early from a stay in hospital, Royal Freemasons operate a Transition Care service that is available both to clients within a residential setting and those who choose to return to their home. The Community Transition Care program provides restorative services designed to enable clients to continue their recovery out of the hospital system, and arranges the appropriate long term care, should it be required. Services include home nursing, domestic home help, case management and transport support, all of which are delivered by fully trained members of Royal Freemasons' experienced transition care staff.

Day Therapy Centre

Located within the Coppin Centre adjacent to St Kilda Road, and operated by fully qualified health professionals, Royal Freemason's Day Therapy Centre provides a wide range of therapy and rehabilitation services for elderly people, whether they are residing in aged care accommodation or living at home.

Day therapy programs focus on restoring independence, confidence and self esteem with the provision of short term rehabilitation following illness or injury. Special programs are developed following an individual assessment and could include podiatry, physiotherapy, speech therapy, optometry, audiology and occupational therapy.
